Ultrasound Referral Form

As the outpatient ultrasound service is an extension of your practice, the referral partner is responsible for communicating the ultrasound findings to the client. Trophy Club Animal Hospital will email you the radiology report to the address provided in the Outpatient Ultrasound Referral Form.

Referring Clinic Information

The imaging report will be emailed to this address.
